1. Service Description & Scope The Virtual Skincare Audit is a free, non-medical service provided solely to offer personalised homecare recommendations using AlumierMD products. It involves review of your submitted information, photographs and routine details, followed by a bespoke prescription created on the AlumierMD Connect platform. This service does not constitute medical advice, diagnosis, clinical treatment, or a professional consultation under healthcare regulations. It does not create eligibility, priority or access to in-studio skin treatments at Samantha Wade Skin Studio, which remains closed to new clients.

2. Eligibility & Frequency You must be 18 years or older to submit a Virtual Skincare Audit form. This complimentary service is strictly limited to one audit per individual per calendar year. Multiple submissions within the same year may be declined without notice.

3. No Charge The Virtual Skincare Audit and resulting AlumierMD prescription are provided completely free of charge. No payment is required at any stage unless you wish to purchase the recommended products.

4. Submission Requirements By submitting the form you confirm that:

   • All information provided (including photographs) is accurate, current and relates to yourself.

   • Photographs are unfiltered, taken in natural light and show the relevant areas truthfully.

   • You have disclosed any relevant medical conditions, allergies, pregnancy/breastfeeding status, or medications that could affect skincare recommendations.

5. AlumierMD Connect Registration To receive your personalised prescription, you must complete registration on the AlumierMD Connect platform (https://connect.alumiermd.co.uk). This includes verifying your email via the link sent by AlumierMD. Failure to register and verify will prevent issuance of your prescription, as I cannot access or create prescriptions without a verified AlumierMD Connect account linked to you. AlumierMD's own terms of use apply to your account and purchases.

6. Data Protection & Privacy All personal data and photographs submitted are processed in strict compliance with the Data Protection (Jersey) Law 2018.

   • Lawful basis: Consent (given by submitting the form) and legitimate interests (providing the requested service).

   • Purpose: Solely to deliver your Virtual Skincare Audit and AlumierMD prescription.

   • Storage & security: Data is held securely and access is restricted to Samantha Wade.

   • Retention: All submitted data and photographs will be deleted no later than 90 days after your prescription is issued (or sooner if you request deletion).

   • Sharing: Data may be shared only with AlumierMD (via Connect platform) to fulfil your prescription, or where required by law. No other third-party sharing occurs without your explicit consent.

   • Your rights: You have the right to access, rectify, erase, restrict processing, or object to processing of your data. Contact info@samanthawade.co.uk to exercise these rights.

   • Complaints: You may contact the Jersey Office of the Information Commissioner if you have concerns about data handling.

7. Recommendations & Liability Recommendations are based solely on the information and images you provide and reflect my 26+ years of experience as an advanced aesthetician. Results vary depending on adherence, lifestyle and individual factors. No guarantees are made regarding outcomes. Samantha Wade Skin Studio accepts no liability for any adverse reactions, dissatisfaction, or consequences arising from use (or misuse) of recommended AlumierMD products. You are responsible for patch-testing products and discontinuing use immediately if irritation occurs.

8. Intellectual Property Any advice, prescription, or written plan provided remains the intellectual property of Samantha Wade Skin Studio. You may use it for personal purposes only and may not reproduce or share it commercially.

9. Changes & Termination We reserve the right to modify, suspend, or discontinue this service (or these terms) at any time without notice. Continued submission after changes constitutes acceptance.
